Student Athlete Enrollment and Clearance Form

Parents of TPHS student athletes for the 2026-27 school year:
Please submit the Student Athlete Enrollment and Clearance Form if your student plans to participate in sports next school year at Tellico Plains High School. Submitting your contact information will allow Candace Yates, TPHS Athletic Trainer, to email parents a link to sign up for athletic paperwork. The information completed by the parent will be maintained by the Athletic Trainer provided by Prisma Health and Blount Memorial Hospital. Click title to access this form.
